This Incident Investigation, Root Cause Analysis (RCA) and Problem-Solving course include the theoretical and practical components to meet the skills and knowledge required to undertake investigations as part of the job role for technical personnel and managers.
Participants will gain the following skills and knowledge:
- Solving routine and non-routine problems
- Correct investigation techniques
- Drilling down to the root cause
- Proposing solutions to address root causes
- Completing incident investigations
Training and assessment are conducted at Site Skills Training facilities. Upon arrangement, training and assessment can be conducted at the premises of industry clients providing there are suitable industry training facilities.

This course is suitable for supervisors, managers, engineers and technical personnel who are required to investigate incidents, conduct a root cause analysis and solve problems.
Participants are required to bring at least two (2) recent incidents from their workplace that will be used for their in-class activities.
A moderate English language, literacy and numeracy skill level is required.
All participants are required to wear the following minimum PPE to enter our facilities:
- Fully enclosed shoes (steel-capped boots preferred)
- Long-sleeve shirt with high visibility
- Long work pants
Additional PPE may be required depending on the course requirements.
Upon successful completion of this course, a nationally recognised Statement of Attainment for the following units of competency will be issued:
